The Licensed Practical/Vocational Nurse works under the direction of the Medical Director of the Canton CBOC Mental Health Clinic and designee in collaboration with designated Nursing Supervisor and designee. The LPN/LVN is responsible for observing, recording, and reporting changes in the patient's condition, as well as assisting in the development and revision of the patient-driven treatment plan. [ "VA Canton CBOC is an interdisciplinary outpatient facility within the VA Ann Arbor Healthcare System, serving medical, surgical, and neurological outpatients. There is a large outpatient psychiatric population. The work is performed in the CBOC Mental Health Clinic areas and incumbent must be able to travel within/between clinical areas with ease as needed. This may also include travel to other sites within the VAAAHS as necessary in the performance of the incumbent's duties. A significant amount of time is spent working at a desk utilizing the computer or phone. It involves the frequent use of office equipment including the telephone, copier, personal computer, printer and mainframe terminals. The LPN is the technical expert for performing, reporting, and recording assigned nursing care, including, but not limited to: Utilizes basic nursing skills and techniques, of varying degrees of complexity, in a general care setting, to provide personal and general nursing care to adult patients, aged 18-85 and older, who require treatment for common mental health conditions Assists in performing nursing assessment Performs focused screening, assessments, observational, or event-focused assessments Collects and timely communicates data and information Determines immediate need for intervention Provides data to assist in development of a nursing diagnosis Contributes to the development of the nursing care plan Participates in data collection and implementation of care for all aged patients treated Verifies physician's orders by reviewing documentation Provides first aid or emergency treatment as required Implements the identified nursing care plan under RN supervision As a member of a multidisciplinary health care team, conducts patient interviews for assessment of treatment needs Participates in developing, implementing, and evaluating patient care treatment plans Monitors vital signs such as temperature, blood pressure, and pulse Collects and records information regarding patient's physical and psychological condition Summarizes and presents information to the treatment team Provides assistance to providers during appointments Assists the MHC RN in identifying case management needs Refers patients to appropriate resources based on the nursing care plan Completes clinical reminders with patients Utilizes Clinical Video Teleconferencing (CVT) equipment to facilitate patient visits with remote providers as a tele presenter Gathers necessary documents for chart preparation, charts and documents all medications, treatment, vital signs, progress, behavioral changes, and vital statistics (such as height and weight) Cleans and sterilizes equipment and instruments; cleans examination areas; maintains clinic equipment and supplies Provides within scope of practice direct patient care and consultation in the practice of nursing; nursing education to patients, guardians and relatives Assists with care coordination within agency and between the agency and community partners, hospitals, medical and nursing providers Maintains timely and accurate documentation in accordance with federal regulations, state statutes and rules, and agency policies Administers medications (including injections) Participates in patient teaching in both individual and group formats, such as education on medication or side-effect issues Work Schedule: Monday-Friday, 8:00a.m. to 4:30p.m. Financial Disclosure Report: Not required" ]
