Job Description:
The Social Worker, as a member of the interdisciplinary team,
provides services in response to the emotional needs of residents
and their families. The Social Worker also assists with resident
admissions and the referral processDUTIES:Maintaining and/or
improving the resident's ability to improve everyday physical needs
and mental/psychosocial needsIdentifying individual social and
emotional needsDeveloping and Maintaining individualized care
plansParticipating in the planning of the resident's admission,
community re-entry or transfer to another facilityParticipating in
interdisciplinary care plan meetings to ensure treatment of the
social and emotional needs of the residentREQUIREMENTS:MI State
Social Work license is requiredBachelor's Degree in Social Work or
PsychologyLong Term Care / Subacute Experience Required
